What it does

Which medical groups are losing clinicians? Windows two CMS reassignment snapshots (individual→group affiliation): per group, clinicians (NPIs) affiliated in the prior snapshot but gone in the latest. Filter by state, specialty, org (group name), group_id, min_dropped, or a since baseline. Returns net churn per org + the dropped NPIs — the facility-losing-clinicians backfill tell.
